Denied claim

An insurer's decision that a loss is not covered or not payable under the policy. It can follow from an exclusion, a lapse in coverage, or a limit that is already used. A denial can often be challenged through a policy appeal, an appraisal process, or a complaint to the state regulator, and documentation carries the challenge.
